FILING REQUIREMENTS FOR A UTILITY MODEL APPLICATION

1. Formal Information 
    The following information is required for filing a formal application:
    - Name and domicile of the applicant
    - Name and domicile, or place of business of the agent, if designated
    - Date of filing the application
    - Title of the device (invention)
    - Name and domicile of the (device) inventor

2. Specification 
    A specification, a drawing(s), and an abstract shall be attached to the utility model application, including:
    - Title of the device;
    - Brief explanation on the drawing(s);
    - Detailed description of the device; and
    - Claim(s)

CONVERSION APPLICATION

An applicant for a patent, utility model, or design application may convert his application into any of the other two 
applications within specified time limits if the original application was filed before July 1, 1999. However, this system does not 
apply to those applications filed on or after July 1, 1999.
